When a manuscript is submitted to the Journal of Visual Impairment & Blindness (JVIB), the corresponding author is required to complete a number of forms related to copyright and permissions.
Forms Included in This SectionJVIB Photo Release Form (Subject)
If a manuscript includes a photograph of an individual, a release form needs to be completed by the subject of the image that provides JVIB permission to publish his or her likeness.
JVIB Photo Release Form (Photographer)
If a manuscript includes a photograph that was not taken by the manuscript's author, a release form needs to be completed by the photographer that provides JVIB permission to publish the photograph.
Permission to Reprint Material in JVIB Form
If a manuscript includes any material that has been published elsewhere—for example, a bar graph, form, table, or chart—a release form giving JVIB permission to reproduce the material needs to be signed by a representative of the original publisher of the information.
